Speakers who have submitted their proposals through the Call for Proposals will be notified of their acceptances in September 2008. For those accepted, the following forms will be due November 15, 2008.

Commerical Support/Interest Disclosure Form
Mandatory Financial Disclosure
Documentation of FDA Status
All Syllabus and slide presentations are due March 1, 2009.
