2012 AAOS Presenters

The following forms are due to the NAON National Office by
Friday, July 15, 2011:
 

*Failure to return these forms will result in the delay of syllabus production and may jeopardize NAON's educational efforts in the future.*

 

 

 

  • Presenter Acceptance Form
  • Biographical Data Form

    The following forms are due to the NAON National Office by Monday, August 1, 2011:

  • Activity Outline Form  (This form MUST be emailed to the NAON National Office in a word document. List learner objectives and presentation outline. Objectives should be the same as on the speaker acceptance form.)
  • Audio Visual Requirements/Presenter Release Form
  • Conflict of Interest Form  (This form clarifies NAON's position about referring to commercial products during continuing education programs.)
  • Documentation of FDA Status  (This documentation form is needed for NAON to comply with FDA regulations.)
  • W9 Form (We need this in order to submit your honorarium)

    Syllabus Materials Thursday, December 1, 2011
    **Download the Conflict of Interest Slide. You must include the Conflict of Interest slide as your first slide after your title slide in your presentation and handouts.**

    2012 Congress Presenters

    The following forms are due to the NAON National Office by  Monday, September 9, 2011:

  • Presenter Information Form

    The following forms are due to the NAON National Office by November 1, 2011

  • Presenter Acceptance Form
  • Activity Outline Form  (This form MUST be emailed to the NAON National Office as a word document. List learner objectives and presentation outline. Objectives should be the same as on the speaker acceptance form.)
  • Audio Visual Requirements/Presenter Release Form  (Indicate AV requirements so we can adequately prepare for your AV needs. Laptops not provided.)
  • Biographical Data Form  (Required of all participants for use by session moderators to create a speaker introduction.)
  • Conflict of Interest Form  (This form clarifies NAON's position about referring to commercial products during continuing education programs.)
  • Documentation of FDA Status  (This documentation form is needed for NAON to comply with FDA regulations.)
  • W9 Form (We need this in order to submit your honorarium)
