Hi everybody! I have a question about how everyone documents the use of K wires. The question was asked today about how to document the K wires when they are used temporarily and removed. I understood that they would be documented as used and removed on the record. An orthopedic surgeon then told me that documentation like that increases the risk of liability. ?? What are your thoughts on this? How do you document if at all?

I would suggest asking your Risk Management staff. It seems odd that the nurse wouldn't document there presence and condition. We document all devices, equipment, etc in the shift assessment along with the status, ie off, on, pin site assessment, etc.
