Hi all!

My facility is switching to a new computer charting system, which has given us the much needed opportunity to clean up and streamline our charting. When we looked at our charting for M/S, we noted that it is very subjective and very inconsistant. I.E., what one nurse charts in her assessment on the am shift may be charted differently by the pm shift.

Does anyone know of a good assessment tool for the musculoskeletal system that we could consider? Ideally, we'd like one that is on some sort of a scale.
