Annual Reports Due April 1st
The Chapter Advisory Team and National Office want to know how your Chapter is doing. Fill us in with your completion of each of the forms. Remember, in order to receive a reimbursement check, you must submit all forms by April 1st and have a minimum of 6 education hours during 2007. All Chapters must complete and submit the Annual Reports per Policy 6.4
You can access the forms and information in this email and at this link. Save your reports as Chapter##-OfficerRoster or Chapter##FinancialReport etc. Reports must be e-mailed to Jessica Daniels ( jdaniels@smithbucklin.com ) and your CAT Representative (contact information available here)

IRS Reporting
All Chapters are responsible for filing Form 990 with the Internal Revenue Service. To complete this form, you will use the same information you have listed on the 2007 Chapter Financial Report.

* Complete the information on both tabs of the Excel Workbook.
* The form is designed with “macros” in place. If asked during the download – accept the macros. These will allow the sheet to automatically calculate all the figures.
* If you are unsure of the information requested in each box, putting your mouse arrow in the box will show a “hint” box.
* Fill in “0” where appropriate
* If your Chapter gave any scholarships or grants in 2007, please complete the Report of Scholarships form as well
* The information you provide on these sheets will be the same information needed to complete the IRS Form 990.
